People they may see include:

  • family and friends of people with diabetes
  • people at risk of getting diabetes.

The team includes a:

  • Doctor
  • Dietitian
  • Podiatrist
  • Social Worker
  • Exercise Physiologist

What can they help with?

  • How to check you blood glucose levels.
  • Information about diabetes and its treatments.
  • How to self manage on your own.
  • Setting goals to help you manage your diabetes.
