My Aged Care Services

Stawell Regional Health is a My Aged Care Service Provider delivering the following services under My Aged Care:

Memory Support Nurse

For many of us, as we age, we notice differences in our ability to remember things. This is often a normal part of aging, but sometimes it is more serious and concerning, (for example if you have dementia). Robyn Leslie, Stawell Regional Health’s Memory Support Nurse can help you with strategies to support your memory, make referrals to doctors, or other services who will help you with your memory and can work with your carer or family member to support you.

Referrals to the Memory Support Nurse must be made through My Aged Care. Please contact us if you need assistance with My Aged Care or you have questions or concerns on 03 5358 8546

Social Support Group

Getting older doesn’t have to mean becoming isolated, but for some of us, there are many reasons why it might be hard to stay connected with the community. Our Social Support Group has a supportive, friendly and experienced group of staff and a wonderful group of participants, all of whom enjoy themselves every week with activities, exercises, trips, lunches and much more. the short video below gives you a glimpse of what you can expect from the Social Support Group.

Referrals to the Social Support Group must be made through My Aged Care. Please contact us if you need assistance with My Aged Care or you have questions or concerns on 03 5358 8546
