A formal complaint may be more involved than general feedback provided on this form. If you would like to make a formal complaint, please write a letter to the Chief Executive Officer (CEO) or the Health Complaints Commissioner outlining your concerns. They will look into all formal complaints. The CEO will respond in writing following the review on how your complaint has been handled. We aim to resolve most of these within 35 days.

Health Complaints Commissioner (HCC) – Victoria

Any problem is usually best solved at the point of service in the hospital. However, if you are not satisfied with the response to your complaint or you wish to take your concerns further, please contact the Health Complaints Commissioner (HCC). The HCC responds to complaints about health services and the handling of health information in Victoria.  Their services is free, confidential and impartial.  To lodge a complaint with the HCC:
