Hospital Admission Risk Program (HARP)
This program aims to help people and their carers to be safe and independent at home. By meeting their health needs it is hoped to reduce their admissions to hospital. The HARP nurse works with people and carers to understand and manage their illness themselves. The nurse visits people in their home and talks about their medications, their symptoms and generally how they manage their own care needs.
HARP service assists people who may have:
- Risk of a repeat admissions to hospital
- Chronic Lung Disease
- Complex needs which require a team approach
- Chronic Heart disease
Referrals can be made from:
- Your doctor and specialist
- Yourself or your family
- Other health services
- Acute care through a hospital
